Re: Name Correction for <Foreign Spouse's full name>, A<Foreign Spouse's A#>, SSN <Foreign Spouse's social security number>
Dear Sir or Madam,
In September 2002 I sent you a letter indicating a change of address. The address change was completed quickly, which I thank you for. However, my first name was apparently mistyped into the system as "Kaniel" instead of "Daniel." When I received the confirmation of the address change, I assumed the mistake was a one time mistake. However, I recently received further correspondence with the mistyped name.
I would appreciate it if you could amend my files to restore my name to "<Foreign Spouse's Full Name>." For reference, my A# is <Foreign Spouse's A#>, and I am currently awainting Adjustment of Status, which was received by your office on October 19, 2001.
If you have any further questions, I may be reached during the day at <Foreign Spouse's work number> or in the evening at <Foreign Spouse's home phone number>.
Thank you for your time,
<Foreign Fiancé(e)'s full name>
